Creating a network of family support for individuals with developmental disabilities and their families

 

The FAMILY CONNECTIONS project is designed to listen and respond to the needs of individuals with developmental disabilities and their families. Its goal is to access and integrate existing services and determine the need for additional supports in ways that maintain and strengthen each family. Staff work in partnership with family members and individuals recognizing and supporting their strengths and respecting their privacy.

What services does FAMILY CONNECTIONS offer?

•  Information and referral to a variety of community and statewide resources (e.g. medical, legal, financial, educational, vocational and leisure activities).
•  Educational planning and support.
•  Community and systems advocacy.
•  In-home assistance or consultation to provide support and information to the family or the individual who has a disability 
to develop and expand strengths and abilities.

•  Parent to parent networking
•  Limited financial assistance 
 
How is support provided? 
•  Support is provided in an individualized and personal way, building on strengths, respecting cultural diversity, values and unique life styles.
•  Staff work in partnership to creatively find, access or provide the support and services that families and individuals want and need most.
•  Any family member can call us directly or ask to be referred by an agency. 


Who is eligible for FAMILY CONNECTIONS?

•  Biological families, adoptive parents or guardians who provide a home for a person of any age with a disability.
•  The child or adult must meet the eligibility criteria for the Department of Mental Retardation. 
 
          
Support is offered on an "as needed" basis, according to requests, at no cost.
